Smart Discharges

Smart Discharges is a digital health program of research that uses individual-level risk prediction and intervention to increase effective health seeking behaviour, improve health outcomes, and reduce child mortality during the post-discharge period. By integrating risk-prediction models into digital technology, health workers are able to identify children who are vulnerable to recurrent illness, re-admission, and death after discharge at the time of admission. Health workers mitigate risk by educating caregivers on post-discharge care and by scheduling follow-up visits for vulnerable children in their communities.
